Casino operator MGM Mirage Inc. acknowledged that the paperwork for the metal work that caused the fire “did not meet our corporate standards” and said in a statement that the situation was “being immediately addressed.”

 

Fire officials point to cutting torch in Monte Carlo casino-hotel fire

A fire at the Monte Carlo casino-hotel was caused by flying molten metal from a hand-held cutting torch used by workers who did not have proper permits, fire officials said last week.

Workers were cutting corrugated steel to set up window-washing equipment at the casino on the Las Vegas strip but did not use proper mats to protect the roof, the Clark County Fire Department said. The first 911 call came from a passer-by.

If the contracting company, Union Erectors LLC, had applied for a permit, it would have been advised how to perform the work safely and likely been approved in four to five weeks, said Deputy Fire Chief Girard Page.

“It does take time and effort, but it’s not that difficult,” Page said.

Officials are reviewing whether to cite the contractor, which could result in fines of US$1,000 and up to six months in jail per citation.

An attempt to call Union Erectors’ Las Vegas office was unsuccessful.

The melted steel started a fire on the southwest corner of the main facade of the roof of the 3,000-room, 32-story hotel-casino.

Union Erectors had only a permit from the county to install window-washing equipment at the hotel, but not to conduct work with torches. A complete report on the investigation was still being compiled.

It was the first of two roof fires within a week at U.S. gambling resorts. A blaze at the Great Cedar Hotel at Foxwoods Resort Casino in Mashantucket, Conn., also injured no one but forced guests to evacuate. That fire was traced to a malfunctioning device meant to keep pipes from freezing.

The Monte Carlo, which opened in June 1996, is on the Las Vegas Strip not far from the scene of Nevada’s deadliest fire, a Nov. 21, 1980, blaze that killed 87 people at the old MGM Grand hotel and led to strict fire codes in Las Vegas resorts.

This makes the whole industry look bad.

Some businesses in the KY3 area may have fire extinguishers that don’t work because the company that serviced them didn’t do its job properly. The Missouri attorney general’s office says customers of Missouri Fire Extinguisher of Cabool should have their extinguishers checked to see if they have enough powder or pressure.

The owner of the company, which doesn’t appear to be registered with the Missouri secretary of state’s office, is Fred McNew. The Laclede County prosecuting attorney charged McNew with two misdemeanor counts of theft on Oct. 14 after an investigation by the Lebanon Police Department. The charges say McNew failed to properly service two fire extinguishers last January but state officials believe there may be many more victims.

A reporter couldn't reach McNew or his company for a comment because there's no telephone listing or record of the company. Officials at Cabool City Hall and Cabool Area Chamber of Commerce said they have no knowledge of the company. His attorney, public defender Stacy Patterson of Lebanon, is in court and hasn't been available for a comment. McNew had to post a $5,000 bond. His next court appearance is set for Dec. 6.

Among the 49 serious citations, the company was cited for blocked, unmarked or unilluminated exits; blocked fire extinguisher access; no fire extinguisher for a battery charging area; a defective powered industrial truck; crane safety defects; numerous instances of unguarded machinery; a variety of electrical hazards; unsecured compressed gas cylinders; unbonded containers of flammables; uncleaned paint spray booth walls; defective respirators; inadequate personal protective equipment training and use; no respirator program; and no welding shield.

Shoppers were back to their normal routines on Monday at the Macedonia Commons Wal-Mart store, 8160 Macedonia Blvd.

On Sunday at around 8 p.m. the Macedonia Fire Department responded to a fire in the housewares department of the store.

According to Macedonia fire officials, the fire crew arrived to find the store evacuated and the rear of the store filled with smoke.

The fire was extinguished by store employees with two dry chemical extinguishers.

Firefighters confirmed that the fire was out and that there was no threat of reigniting..

Lt. Paul Celinski, with the Macedonia fire department, reported that the store was closed for a short period on Sunday.

An east Charlotte middle school was vandalized over the weekend and police are searching for the people responsible.  According to reports, the suspect or suspects broke into Albemarle Road Middle School, took a fire extinguisher and damaged the hallway and several classrooms. They also broke some windows and threw trash all over the school.

"We can't find anything missing. Things that would be of value…a palm pilot or digital camera. They're still here,” says Calvary Baptist Church pastor Danny Davis. But what they did cost them a lot more than petty cash as windows were broken and both churches were sprayed throughout with their own fire extinguishers covering everything.

"Well, we had to cancel our Wednesday service because this is a bit of an irritant,” Davis says. From expensive musical equipment to floors, everything was covered in both church sanctuaries.

"Apparently the perpetrators got into some of the buses and activated the fire extinguishers," Tempest Bova said. "There was white dust everywhere, making the buses unsafe and inoperable."

Of the district's 128 schools, almost 50 did not pass inspections due to various problems, Seavert reported.  Fire inspectors found door hardware that was sometimes broken, blocked or locked.  One of the most common violations was fire extinguishers that expired.  In addition to schools missing monthly fire drills, 10 Investigates' records showed some schools did not post evacuation or exit plans which could cause confusion in case children file out fast in an emergency.

In an update by Mr. Bartos, he expresses concern about fires, because the alarm system and virtually all fire extinguishers were destroyed in the riot.  He reports keys still missing, and no clear picture of what tools might be gone.

Late Friday night, I-Team 8 received a response from the Indiana Departemnt of Correction.  It said that even before Arizona inmates arrived in New Castle, there was concern that the prison there would not be ready to accomodate so many prisoners so quickly.

The State Department of Correction says to expect a complete report on the riot next week.
